Information Request Application Form

As the Dr. Kerem Kınık Vision Page protection of the privacy of private life, particularly protection of fundamental rights and freedoms, ensuring and protecting information security and respect for ethical values are among our priority principles. Article 11, “The Rights of Data Subject” of the Law on the Protection of Personal Data No. 6698 (“KVKK”) entitles the data subjects to submit certain requests with regards to the processing of their personal data. As the Data Controller, the Dr. Kerem Kınık Vision Page has prepared this information request application form in order to ensure that the Data Subjects can exercise such rights, and to fulfil the disclosure liability set forth in Article 10 of the KVKK.

Pursuant to Article 11 of the KVKK, you may exercise the rights below by applying to the Dr. Kerem Kınık Vision Page and fill out the attached form:

  1. You can learn whether your personal data is processed or not,
  2. You can request information if your personal data is processed,
  3. You can learn the purpose for processing your data and whether such data has been used for intended purposes,
  4. You can learn the third parties to whom your personal data is transferred in the country or abroad,
  5. You can request the rectification of your incomplete or inaccurate data, if any,
  6. You can request the erasure or destruction of your personal data under the conditions laid down in Article 7 of KVKK,
  7. You can request notification of the operations carried out in compliance with subparagraphs (e) and (f) to third parties to whom personal data has been transferred,
  8. You can object to the processing, exclusively by automatic means, of your personal data, which leads to an unfavourable consequence for you,
  9. You can request compensation for the damage arising from the processing of your personal data in a way that is in violation of the Law on the Protection of Personal Data

Pursuant to Article 13 of the KVKK, the data subjects shall lodge an application in writing to the Dr. Kerem Kınık Vision Page or via other methods specified by the Board (“Board”). Within this scope, applications to be submitted to us can be lodged free of charge, taking a printout after filling out this form hereby, and by choosing one of the three methods specified below.

Via Registered Electronic Mail (KEP) signing with

the “Secure electronic signature” as defined in the Electronic Signature Law No. 5070

 

Subject of the mail must be “Personal Data Protection Law Information Request”.

Third parties may not exercise the right to obtain information on behalf of data subjects, governed in Article 11 of the KVKK. In order for the data subjects to submit a request about the personal data of a person other than themselves, an original signature-bearing and notarized original of the special power of attorney issued by the data subject in the name of the person to submit the application must be presented.

Pursuant to Article 13/2 of the KVKK, your applications submitted to us will be answered within thirty days as of the date of receipt, in accordance with the nature of the request. Our answers will be delivered to you in writing or in electronic medium in line with Article 13 of KVKK.
